Medical History Form
Before your first session, you’ll be asked to complete a medical history form — an important step that helps us create a safe and personalized experience for you.
This form gathers basic client information, including your name, date of birth, occupation, how you found us, and your emergency contact. It also includes a short health overview covering body systems such as:

SOAP Notes
Every massage session is carefully tracked using SOAP notes, a professional method that helps your therapist monitor your progress and tailor each session to your body’s unique needs.
Subjective (S): We start by noting any problem areas or discomfort you’re experiencing. This allows us to focus on what matters most to you during your session.
Objective (O): During the massage, your therapist records which areas of the body were worked on, the techniques used, and the level of tension felt in each muscle group. This ensures that each session is specific and intentional.
Assessment (A): After the session, we summarize where the most tension was found in your body. This helps both you and your therapist understand patterns of tightness and areas that need ongoing attention.
Plan (P): Finally, your therapist provides guidance on which areas to stretch, tips for home care, and recommendations for your next massage to maintain balance and relieve tension.
